Form to Request Event Space
This Request form initiates the reservation of your meeting space. It also communicates equipment needs and, for church events, it communicates details that can be used for church news. Your completed form is shared with the pastor, the administrative assistant, the sexton, and communications staff. Please make your request at least thirty (30) days prior to your event. Please note that there is a cost associated with room usage for public events. All building use and building charges are confirmed through the Administrative Assistant's office (217-528-4311, ext. 204).
